At the end of October Bayview will no longer be collecting monthly strata fees on our behalf and we will be moving our accounts to Coast Capital Savings. Therefore we require new Pre-Authorized Debit (PAD) forms from all owners OR ten post-dated cheques for your monthly fees made out to: The Owners, Strata Plan VIS 5801.

This is a fillable PDF form, which you can download to your computer, complete using your keyboard, save, print, sign and return to me, either by email or drop it off at unit #21.
